Vasectomy — the most effective male contraception

Vasectomy is a microsurgical procedure that divides or occludes the vas deferens — the tubes that carry sperm from the testes to the urethra. After the procedure the body continues to produce sperm, but they no longer reach the ejaculate. Effectiveness is 99.9% — the highest rate of any contraceptive method, for either sex.

No-scalpel vasectomy (NSV)

At Femi Premium we use the no-scalpel vasectomy (NSV) technique, developed in China in the 1970s and now recognised as the gold standard worldwide. Instead of a traditional incision, the surgeon makes a tiny puncture in the scrotal skin with a specialised instrument and gently separates the tissue. The result:

• No sutures — the puncture heals on its own within 2–3 days
• Minimal bleeding — significantly lower haematoma risk than conventional methods
• Less pain — less tissue disruption, faster return to activity
• Near-invisible scar — a mark of approximately 2–3 mm

Each vas deferens is also ligated, a short segment is excised, and the ends are cauterised — minimising the risk of spontaneous recanalisation.

Step-by-step procedure

1. Qualifying consultation — interview with the urologist, physical examination, blood test orders (CBC, coagulation panel, HBsAg), full discussion of the procedure
2. Day of procedure — arrive having shaved the scrotal area (ideally the day before). The procedure is performed under local anaesthesia using a modern needle-free injection technique
3. Procedure (approx. 20–30 min) — the urologist locates both vas deferens, makes the puncture, then divides and seals them. You remain awake and may feel mild tugging — not sharp pain
4. After the procedure — 30 minutes of observation, post-operative instructions, home the same day
5. Follow-up semen analysis — after 3 months (or 20 ejaculations), a spermogram confirms the absence of sperm. Additional contraception is required until then!

Pre-procedure preparation

At the qualifying consultation the doctor will order:

• Full blood count (CBC)
• Coagulation panel (APTT, PT, INR)
• HBsAg (hepatitis B surface antigen)

On the day of the procedure:

• Shave the scrotal area (ideally the evening before)
• Wear comfortable, loose underwear and trousers
• Bringing a companion for the drive home is recommended but not required
• Fasting is not necessary

Recovery — what to expect

No-scalpel vasectomy has one of the shortest recovery times of any urological procedure:

• Days 1–2: Rest at home, ice packs (15 min every hour), over-the-counter pain relief as needed. Most men describe the sensation as “mild pulling” — not sharp pain
• Days 3–5: Return to desk work and light activity. Avoid heavy lifting
• Week 1: Return to physical work; light sport (walking, stationary cycling)
• Week 2: Full activity, including sexual intercourse
• Month 3: Follow-up semen analysis confirming the procedure’s effectiveness

Vasectomy and sexual function

This is the most common concern — and the easiest to address. Vasectomy has no effect on:

• Erections — the erectile mechanism is neurovascular; the vas deferens plays no role in it
• Libido — the testes continue to produce exactly the same amount of testosterone as before
• Ejaculation — it looks and feels identical. Sperm accounts for only ~2% of ejaculate volume; the rest is secretion from the prostate and seminal vesicles
• Orgasm — sensations are unchanged

Research shows that many men report an improvement in their sex life after vasectomy — because the anxiety around unplanned pregnancy disappears.

Effectiveness and safety — the numbers

• Effectiveness: 99.85–99.9% (Pearl Index: 0.1–0.15)
• Procedure time: 20–30 minutes
• Complication risk: <2% (mainly minor haematoma or swelling)
• Spontaneous recanalisation risk: 0.03–0.05% (1 in 2,000–3,000 procedures)
• Infection risk: <0.5%
• Post-vasectomy pain syndrome (PVPS): 1–2% (chronic scrotal discomfort, usually resolving within a few months)

For comparison: the contraceptive pill has 91–99.7% effectiveness, and condoms 82–98% with typical use.

Contraindications

Vasectomy is a minimal-risk procedure, but relative contraindications include:

• Active scrotal skin or urinary tract infection
• Coagulation disorders (require further assessment)
• Hydrocele or varicocele (may require concurrent treatment)
• Uncertainty — if you are not 100% sure, we will not perform the procedure

There is no age limit — the procedure is available to adult men of all ages.

Legal status in Poland

Vasectomy as a reversible contraceptive method has never been prohibited or criminalised in Poland. Article 156 of the Penal Code addresses permanent deprivation of reproductive capacity — vasectomy, being potentially reversible (via vasovasostomy), does not fall under that provision.

The patient signs an informed consent form confirming knowledge of the procedure, its alternatives, and potential outcomes — standard practice for any medical procedure.

Vasectomy and reversal — can it be undone?

Vasovasostomy (vasectomy reversal) is technically possible, but:

• Success rates decline over time — within 3 years of vasectomy, the chance of restored fertility is approximately 75%; after 10 years it falls to around 30%
• The reversal procedure is significantly more expensive and complex than the original vasectomy
• It is not guaranteed — even if the vas deferens are reconnected, fertility does not always return

Treat vasectomy as a permanent decision. If you are not 100% certain — wait. There is no rush.
